Forms Needed for Tryout
Valid Physical – Exam date must be May 1st 2008 or later to tryout and May 1st 2009 or later to be valid for the next school year. Results of physicals must be entered on the four-page Middle School League form (pdf). These are also available in the main office at any middle school.

Cheerleading Tryout Candidate Information Form (MS Word) – This form must be completed by the first day of try-outs. A parent signature must be on the form.
